New York Attorney General Letitia James is facing growing scrutiny over her office’s handling of Medicaid fraud after federal authorities moved to decertify New York’s Medicaid Fraud Control Unit and suspend approximately **$60 million in annual federal funding**.
Now, U.S. Senate Aging Committee Chairman **Rick Scott** is demanding answers. In a new letter to James, Scott pointed to federal findings showing dramatic declines in criminal enforcement. According to data cited in the letter, New York secured just **53 Medicaid fraud convictions from 2023 through 2025**, compared with 129 in the next-lowest comparable state. Even more striking, the unit reportedly secured only **four patient abuse and neglect convictions** during those three years despite receiving more than **2,000 allegations annually**.
The numbers began falling sharply after James took office. Scott says Medicaid fraud convictions dropped from **56 in 2019 to 14 in 2020**, while patient abuse and neglect convictions fell from **30 to four**.
Federal auditors also raised concerns about New York’s emphasis on complex civil litigation over criminal prosecutions and failures to reliably track cases referred to outside prosecutors. Meanwhile, the unit reportedly returned only **$1.84 for every taxpayer dollar spent in 2025**, compared with a national average of **$4.64**.
James now faces a **September 30, 2026 recertification deadline**, and Scott wants her office to turn over its corrective action plans and explain what went wrong.
This comes as Republican AG candidate **Saritha Komatireddy** makes Medicaid fraud enforcement a centerpiece of her campaign against James, arguing that New York taxpayers have lost hundreds of millions of dollars in potential fraud recoveries.
